Letter 37: Request for Transition Assessments and Transition Planning
When to Use It
Send this letter early enough for transition assessments to inform the first IEP that will be in effect when the student turns 16. Federal law sets that latest start, unless the team chooses earlier; state law may require an earlier age (34 C.F.R. § 300.320(b)). Use the 50-State Companion for the current state rule.
Send it to: the director of special education, with a copy to your child’s case manager or IEP team chair. Email with a read receipt; keep a copy in the folder.
