When to Use It
Use this shell to build the complaint you file with OSSE's Office of Dispute Resolution once your claim inventory (DC-Tool 2) and your respondent selection (DC-Tool 7) are settled. This is not a fill-in-the-blank substitute for the OSSE model form (rev. 01.12.22); it is the content skeleton the law actually requires: the six federal elements, plus the D.C.-specific additions (parent name and address, the LEA of enrollment versus the LEA you are complaining against, and the school complained against) that 5-A DCMR 3049.2 adds and that the SOP's own summary of complaint contents, Section 405.C, leaves out. File it with ODR and serve the respondent the same day; ODR's receipt date is what fixes the procedural calendar (ODR SOP sections 204, 404, and 406.A, checked August 13, 2026).
